HOW TO APPLY FOR A WORK PERMIT

Download and print the Employment Permit Application for 14 through 17 Year-Olds.
Have your employer complete the Promise of Employment in Step #2.
Students aged 14-15 must have their Physician complete the Certificate of Health in Step #3.
All students must obtain their parent/guardian signature in Step #4 and sign the application themselves in Step #5.
Bring completed application and proof of age (such as passport or birth certificate) to:
- Ms. Laura Damon, Executive Assistant to the Superintendent-Director
- Office: 1st floor of the Administration Building across from the Main Conference Room (MCR)

Bring the signed work permit back to your employer who must keep it until you leave the job.
